1/3/2024 EnchantaVaughn Approved (AM) Upon arrival, Winter Program, client was not compliant or cooperative at the beginning of class. Client was reported acting out in class. Client is struggling with listening and following directions. Client cannot maintain appropriate behaviors throughout the day, especially when faced with distractions. Client is constantly making noises in his seat and being disruptive in class. Client refused to sit on the carpet during reading time. Client stood at the door and pouted for 10 minutes. Client looked at his classmates and yelled âshut upâ. Client was directed to be respectful and not to take his attitude out on other people. Client continued to have a difficult time adhering to classroom management and listening to his counselor. Client ran out of group room and began chasing peers down the hallway. Client was observed horse playing and making inappropriate noises until counselor arrived. Client was confronted about his choices and leaving group room without permission. Client would not process his feelings with QMHP and was escorted to take a 15 minute time out. Client was separated from negative behaviors and given a chance to correct his mistakes. Client outbursts persists for several minutes during his break and additional redirection was provided. Client ran off during one on one to attend art specials. Client outbursts went away and he was at his table working quietly like nothing happened. (PM) Client displayed civilized manners in lunch by sitting quietly with his class and being polite to staff. Client seemed to be stable and content, at this time. Client did not make it out of lunch on a good note and had to be restrained and carried to QMHP for assistance. Client is witnessed stomping, yelling, crying, shaking his arms and legs aggressively, falling out on the floor, pouting, flustered, upset, and being defiant while given redirection. Client ignored prompts and would not listen when directed to get up and sit in his chair. Client continued to crawl on the floor. Parents were contacted immediately. (EOD) Client was observed in group room in the reading station reading a book and laying across the bean bag, displaying appropriate behaviors. Client was on task and focused. QMHP praised client for coming to class and doing what he supposed to do. Client was monitored and supported for exhibiting good social skills and interacting well with others.
